How to Report Lost or Stolen Hardware

1. Submit a police report immediately.

2. Create a ticket at https://samuelmerritt.teamdynamix.com/TDClient/123/SupportPortal/Home/ or call our main support line at 510-907-2555 option 1. Submit as much of the following details as you can:

  • Did your hardware contain any confidential information?
  • Brief description of the lost or stolen event
  • Attach a copy of the police report.

3. Your supervisor and IT management must be notified.

4. (ITS Staff only) Please attach a screen shot of the verified device encryption to the ticket.

Please note that a replacement hardware can't be processed until the above criteria is met and the request is submitted and reviewed.
